<td style="padding-top:15px;"><a title="The IRC in Uganda" href="http://ww.theIRC.org/uganda" target="_self">Uganda's</a> northeastern Karamoja region is set apart from the rest of the country, both by geography and by the traditions of its inhabitants, most of whom are semi-nomadic livestock herders. Throughout the 1980s and 1990s, this isolation kept Karamoja safe from the HIV/AIDS epidemic.Because HIV/AIDS is still relatively new to the region, knowledge and attitudes about the disease lag behind the rest of the country.</p>
<p>The spread of HIV/AIDS within Karamoja is closely related to the frequency of rape during violent cattle-raiding among the region's different clans, as well as during courtship.</p>
<p>Another contributing factor is the still-common practice of bride inheritance, in which newly widowed women are taken as wives by a male member of their deceased husband's family. Where a widow has been infected with HIV by her husband there is a risk that she in turn will unknowingly infect her new husband.</p>
<p>To counter these practices, the IRC provides education and counseling to rape survivors and offers community education programs about the effects of rape and violence against women.</p>
<p>The IRC also sponsors community gatherings where drama groups perform plays and songs with HIV-related educational themes.</p>
<p>"The performances convey to male audiences that rape and abuse of women are flatly unacceptable," says IRC HIV/AIDS program officer Drametu Jimmy.</p>
<p>The performers in the dramas act out of their own life experience - many are HIV-infected themselves.</p>
<p>"I do this to soften the hearts in the community," said Amuge Patricia, a member of a drama group based in Kotido district. "I want them to know that being infected does not make you cursed or a monster."</p>

Die  Kombinationstherapie muss zur PEP vier Wochen lang eingenommen werden. Auch hier  gilt: Medikamente haben Nebenwirkungen. Mal mehr, mal weniger. Mal heftiger, mal  schwächer. Das ist bei jedem anders. Häufig sind die Nebenwirkungen gerade in  den ersten paar Wochen nach Therapiestart besonders heftig, weil unsere  Entgiftungsorgane einige Wochen benötigen, um ihre volle Leistung zu  erreichen.</span></span></p>
<p>Spätestens nach den ersten vier bis sechs Wochen sind die  meisten heutigen Medikamentenkombinationen gegen HIV einigermaßen verträglich;  die PEP wird aber nur in der Zeit verstärkter Nebenwirkungen  eingenommen.</p>
<p>Daher ist die Zahl der Leute groß, die vor Symptomen wie  Übelkeit, Erbrechen, Durchfällen, Bauch- und Kopfschmerzen kapitulieren und die  PEP nicht die gesamten vier Wochen einnehmen. Das sollte man sich besser in Ruhe  vor dem Eintreten eines solchen Notfalls überlegen:<br />

<p class="MsoNormal" style="margin:0;"><span style="font-family:Tahoma;font-size:x-small;">Durch  Fehler bei der Einnahme antiviraler Medikamente kann die  <span style="text-decoration:underline;">Resistenzentwicklung</span> unterstützt werden. Wer bereits unwissentlich  infiziert ist und eine PrEP mit Truvada oder gar mit der Einzelsubstanz Viread  versucht, züchtet Resistenzen (weil für die Therapie der HIV-Infektion stärkere  Kombinationen benötigt werden). Bei der späteren Behandlung der HIV-Infektion  erschwert eine Resistenz gegen antivirale Wirkstoffe die weitere Therapie.  Heißt: <strong>Wenn´s dich dann mal  erwischt, sind in dem Fall einige Medikamente möglicherweise wirkungslos, weil  das Virus sich schon dran angepasst hat.</strong> Wenn man sich also einem  dieser Experimente unterzieht, dann sollte man sich sehr genau an die  Einnahmevorschriften halten. Und man muß sicher sein können, dass man nicht  bereits mit HIV infiziert ist.</span></p>
<p class="MsoNormal" style="margin:0;"><span style="font-family:Tahoma;"><span style="font-size:x-small;">Es  gibt bestimmt Situationen, in denen PEP oder PrEP sinnvoll zu nutzende  Möglichkeiten sind. <strong>Ein  gleichwertiger Ersatz für das Kondom und Safer Sex bei freiwilligen,  selbstbestimmten sexuellen Kontakten sind sie mit großer Sicherheit nicht.<a href="#0"><img class="menu" src="http://tbn0.google.com/images?q=tbn:iqaLRIhr-11z1M:http://www.wankelje.de/bilder/pfeil-oben.jpg" border="0" alt="nach oben" width="16" height="12" align="bottom" /></a></strong></span></span></p>

Τον τελευταίο καιρό με ζαλίζουν πολύ τα χάπια.

<p>Τον τελευταίο καιρό με ζαλίζουν πολύ τα χάπια.</p>
<p>Δεν ξέρω γιατί.</p>
<p>Τις πρώτες μέρες που είχα ξεκινήσει τα χάπια με ζάλιζαν αλλά μόνο για τις πρώτες 2-3 μέρες. Τους τελευταίους 5 μήνες δεν είχα πρόβλημα με ζαλάδες και ξαφνικά τον τελευταίο μήνα σχεδόν κάθε βράδυ, μετά από μισή ώρα που έπαιρνα τα χάπια, άρχιζε μία τρομερή ζαλάδα και 2 - 3 φορές είχα και εξάψεις.</p>
<p>Ξέρω ότι είναι μια από τις βασικές παρενέργειες αλλά απορώ που με ενοχλούν τα φάρμακα 5 μήνες μετά και όχι στην αρχή. Εντάξει δεν λέω να έχουν κάποιες παρενέργειες, φάρμακα είναι και τα παίρνεις κάθε μέρα για πολύ καιρό. Φυσικά δεν είναι καλό να παίρνεις φάρμακα για το συκώτι σου και όλα αυτά που μπορούν να σου προκαλέσουν από τις παρενέργειες και από το πολύ διάστημα που θα χρειαστεί να τα παίρνεις. Και φυσικά μία ζάλη δεν είναι και φοβερή παρενέργεια να την έχεις, σε σύγκριση με τα μεγάλα προβλήματα που μπορεί να σου προκαλέσουν τα φάρμακα.</p>
<p>Αλλά όταν είμαι έξω και πρέπει να οδηγήσω και με έχει πιάσει ζάλη είναι επικίνδυνο.</p>
<p>Όταν είμαι με συγγενείς και φίλους που δεν ξέρουν για μένα και εγώ αρχίζω να ζαλίζομαι και να μην είμαι τόσο σταθερός στις κινήσεις μου και στην ομιλία μου και να προσπαθώ να με ελέγξω όσο πιο πολύ γίνετε δεν είναι και τόσο ευχάριστο.</p>
<p>Είμαι με ένα άγχος κάθε μέρα αν θα φτάσω σώος στο σπίτι μου ή αν θα πατήσω κανέναν με το αυτοκίνητο ή αν θα με πάρουν χαμπάρι οι άλλοι ότι κάτι έχω. Αν είμαι έξω και ζαλίζομαι, τότε προσπαθώ να καθυστερήσω να πάρω το αυτοκίνητο και να επιστρέψω στο σπίτι ώστε να μου περάσει η ζάλη.</p>
<p>Η ζάλη συνήθως περνάει μετά από 2 ώρες.</p>
<p>Γενικά πλέον τον τελευταίο μήνα προσπαθώ να μην βγαίνω τα βράδια ώστε να μην χρειαστεί να πάρω το αμάξι μου. Δεν πηγαίνω στο σπίτι τις αδερφής μου για φαγητό, δεν βλέπω τους φίλους μου, τους συγγενείς μου κτλ.</p>
<p>Δεν είναι ότι σίγουρα θα ζαλίζομαι κάθε μέρα από τα χάπια ή ότι θα είναι μεγάλη η ζάλη, αλλά δεν θέλω να μου συμβεί και να είμαι σε δύσκολη θέση.</p>
<p>Τώρα ξεκίνησα και το ένα χάπι που περιέχει μέσα 3 φάρμακα, τα 2 από τα 3 φάρμακα είναι αυτά που έπαιρνα έως τώρα (4 χάπια σύνολο) και περιέχει 1 ακόμα, όλα αυτά σε ένα χάπι.</p>
<p>Μπορώ να πω ότι δεν ζαλίζομαι πλέον τόσο πολύ με αυτό το χάπι.</p>

<p>Δεν ξέρω αν έφταιγε ότι τον τελευταίο μήνα το ένα από τα δύο φάρμακα που έπαιρνα άλλαξε μορφή και από κάψουλα έγινε χάπι. Μπορεί όταν ήταν κάψουλα να αργούσε να περάσει στο αίμα και να μου προκαλούσε την ζαλάδα όταν κοιμόμουν (που γι’αυτό και τα έπαιρνα κατά τις 10:30 το βράδυ, ώστε να μην καταλαβαίνω τις παρενέργειες ΑΝ ΚΑΙ ΕΦΟΣΟΝ υπήρχαν) και όταν έγινε χάπι να προκαλούσε νωρίτερα την ζάλη.</p>
<p>Τώρα βρίσκομαι στο στάδιο του νέου φαρμάκου που προσπαθώ να παρατηρήσω την λειτουργία του.</p>
<p>Είναι 3 βράδια που το έχω πάρει και έως τώρα όλα πάνε καλά σε ότι αφορά την ζαλάδα.</p>
<p>Το «πρόβλημα» που έχω με το νέο φάρμακο είναι η ώρα που πρέπει να το παίρνω.</p>
<p>Πρέπει να το παίρνω 1 ώρα πρίν το φαγητό ή 2 ώρες μετά.</p>
<p>Δύο ώρες μετά δεν μπορώ να το παίρνω γιατί συνήθως τρώω κατά τις 9 με 11και μετά πέφτω για ύπνο. Δεν μπορώ να πέφτω για ύπνο κατά της 12 η 1 το βράδυ και να μην κοιμάμαι απλά και μόνο γιατί πρέπει να περιμένω να πάρω ένα χάπι.</p>
<p>Οπότε παίρνω το χάπι κατά τις 8 με 8:30 ώστε να τρώω κατά τις 9 με 9:30. το θέμα είναι ότι αν έρθει κάποιος να φάμε στο σπίτι ή αν βγούμε για φαγητό, πρέπει να το ψιλοκαθυστερήσω ώστε να το φάω στην ώρα μου και να λειτουργήσει το φάρμακο σωστά.</p>
<p>Ελπίζω μόνο να μην μου προκαλεί και αυτό ζαλάδα ώστε να αποβάλω ένα άγχος από πάνω μου και να μπορώ να βρίσκομαι με τους ανθρώπους που θέλω και πιο αργά και να βγαίνω και λίγο εκτός σπιτιού.</p>
<p>Το καλό είναι ότι πλέον κουβαλάω ένα χάπι μαζί μου και όχι τέσσερα (αν και είναι κάπως μεγάλο στο μέγεθος).</p>
<p>Σε έναν μήνα θα πάω να κάνω νέες εξετάσεις να δούμε πως τα πάω.</p>

I do love me some underage boys. To play with, I mean. And, no, not in that manner. The other manner. Which I will leave to your own interpretation.</p>
<p>I cannot believe that an HIV-positive man in Dallas has been sentenced to 35 years in prison for spitting in the eye of a police officer. Are you even kidding me? This is when I begin to doubt the intelligence of my peers, and whether it is smart to have the general public deciding the fate of people in courtrooms, or choosing our next president. Ehem. Did somebody say George W. Bush?</p>
<p>Have y'all not had to sit through all the PSAs and health filmstrips that I did throughout the '90s? You know, the ones in which we're told over and over and over and over that you cannot get AIDS from saliva? The <a title="CDC on HIV transmission" href="http://www.cdc.gov/hiv/resources/factsheets/transmission.htm" target="_blank">CDC still reports</a> that there has never been a case of HIV transmission through saliva. I am quite sure this officer is not the first person in the world to get an eyeful of spit from someone with AIDS. Not that this kind of behavior should be condoned, but 35 years? Are you for real?</p>
<p>According to the <a title="article on HIV-positive man" href="http://news.yahoo.com/s/ap/20080515/ap_on_re_us/hiv_positive_suspect" target="_blank">article on Yahoo News</a>, the man "used his saliva as a deadly weapon." Is this implying that it was the intent to cause harm rather than the actual perilousness of the act? So were I to think my hugs could kill someone and I embraced an officer while yelling, "I"m going to kill you with kindness, motherfucker!" would that be enough to land me in jail? To quote Chevy Chase in <em>National Lampoon's Vacation</em>, "<a title="You're all fucked in the head" href="http://www.youtube.com/watch?v=vJ8X6qTA5cE" target="_blank">I think you're all fucked in the head</a>."</p>

<p>In dem Text Gedankenleser im Kreuzverhör von Thomas Metzinger werden die möglichen Konsequenzen behandelt, die aufkommen könnten, wenn die Wissenschaft soweit ist, Gedanken durch Maschinen lesbar zu machen.<br />
Nach Thomas Metzinger ist es lediglich eine Frage der Zeit, bis es Wissenschaftlern gelingen wird, die Gedanken lesen zu können, er warnt davor, dass man jetzt schon darüber nachdenken muss.<br />
Metzinger führt einige Wege auf, die Gedanken mittels maschineller Hilfe zu ermitteln und zu deuten, doch keine dieser Geräte, sagt er, sind schon so weit, wirklich zu lesen, was in einem Menschen vorgeht, da sie noch immer ausgetrickst werden können. Sie beruhen alle auf dem Fakt, dass es einem Menschen irgendwie ablesbar sein muss, wenn er vorsätzlich lügt und sich dieser Tatsache bewusst ist.<br />
Am meisten am Erfolg dran ist seiner Meinung nach die Methode „Brain Fingerprint“, dabei werden Probanden mit verschiedenen Gegenständen vertraut gemacht, diese sollen sie später wieder erkennen, während ihnen zusammenhangslos irgendwelche Gegenstände gezeigt werden, unter denen sich auch die Wiederzuerkennenden befinden. Am Erregungsmuster des Gehirns soll dann abgelesen werden, ob die Testpersonen diese Gegenstände schon einmal gesehen haben.<br />
Thomas Metzinger stellt zu dieser Methode die Fragen, ob sie überhaupt zur Wahrheitsfindung eingesetzt werden dürfte und ob sie vor Gericht als Beweis ethisch vertretbar wäre. Ganz nach dem Motto „Der Körper lügt nicht“ erfand Lawrence Farwell diese Methode und spricht von einer fast 100% Erfolgsrate, zu der er allerdings allein durch eigene Test kam. Doch Thomas Metzinger spricht an, dass das „Brain Fingerprinting“ sehr wohl fehlerhaft ist, wenn Probanden sich bei einem ihnen unbekannten Gegenstand an eine sehr starke Erinnerung denken, schlägt das Gehirnmessgerät aus, als ob es den Gegenstand erkennen würde. Vor allem sei es aber nicht ansatzweise zu 100% fehlerfrei, da unter anderem geistig Behinderte oder Drogenabhängige Erinnerungen fehlerhaft speichern und auch verzerrt wieder abrufen und somit ihre Erinnerung nicht der Wahrheit entspricht, sie dies allerdings glauben.<br />
Zu Metzingers Frage, ob das „Brain Fingerprinting“ als Beweismittel vor Gericht zulässig sei, bringt er ein Beispiel, dass es in Amerika schon einen Freispruch gab, aufgrund dieser Methode, da der Verhaftete, der schon jahrelang im Gefängnis gesessen hatte, wichtige Objekte, die nur dem Täter bekannt sein konnten, nicht wiedererkannte.<br />
Allerdings ist es sehr schwierig die gewonnen Daten auszuwerten, da sich das Gehirn eines Menschen im Laufe der Zeit verändert und man auf Gegenstände trotzdem vertraut reagieren kann, obwohl man sie nicht kennt, sie einen aber an etwas anderes, sehr vertrautes erinnern können.<br />
Doch die wichtigste Frage für Metzinger ist weiterhin nicht eine die darauf basiert, dass diese Forschungen schon funktionieren und ob man sie einsetzen darf, sondern wie die Menschheit überhaupt auf einen so gigantischen wissenschaftlichen Fortschritt reagieren soll. Auf jeden Fall steht für ihn fest, dass der Begriff Privatsphäre neu definiert werden müsse. Der Mensch würde auf einmal nahezu undurchsichtig werden und ihm würde jeglicher Schutzraum genommen, den man sich allein durch die Tatsache aufbaut, dass man ohne es anders beweisen zu können lügen könnte.<br />
Man kann folglich die positiven Seiten dieses Fortschrittes sehen, dass z.B. Politiker in ihren Wahlreden nicht mehr bewusst lügen könnten und Wahlversprechen geben, die sie nicht vorhaben einzulösen. Allerdings gibt es viel mehr Nachteile, unter anderem könnten Arbeitgeber ohne Probleme herausfinden, ob eine Frau lügt, dass sie nicht schwanger ist oder ob ein anderer lügt, dass er kein Aids hat und der Arbeitgeber könnte dann diese Menschen, unter Angabe anderer Gründe nicht einstellen.<br />
Es würde wahrscheinlich eine zeitlang dauern, bis es Gesetze gäbe, die es verbieten solche Lügendetektortest durchzuführen, da es genauso lange gedauert hat, bis es Arbeitgebern offiziell verboten wurde, nicht nur die Routineuntersuchung am Blut eines potentiellen Bewerbers durchzuführen, sondern auch seine Gene etc. zu testen, um herauszufinden, ob er z.B. eine Krankheit trägt, die ihn nur weitere 10 Jahre arbeitsfähig macht.<br />
Matzinger führt das weitere Problem an, dass sich Geheimdienste solche Lügendetektoren zu nutzen machen könnten, sie insgeheim weiter entwickeln könnten und auch so Menschen durchleuchten könnten, die gar nichts davon wissen.</p>
<p>Das Gedankenlesen ist seit Urzeiten ein Mysterium, dass Menschen fasziniert, aber auch beängstigt. Immer wieder gibt es Personen, die von sich behaupten Gedanken lesen zu können und dies in Fernsehsendungen wie „Der Gedankenleser“ (im April 2005 auf Sat. 1 erschienen) oder „The next Uri Geller“ (ab Januar 2008 auf Pro7 gewesen) unter Beweis stellen, glauben kaum Menschen so wirklich an solche Fähigkeiten, gerade dadurch, weil es keine wissenschaftlich bestätigen Beweise gibt.<br />
Dagegen spricht das Patent, dass erst im Oktober 2007 von dem Konzern Microsoft eingereicht wurde, der das Gedankenlesen beanstandet. Laut Antrag haben Wissenschaftler von Microsoft es geschafft, „mit der Filtermethode  zuverlässig aus Messdaten Nutzerreaktionen auf die Software wie "Überraschung, Befriedigung und Frustration" abzuleiten“ (Spiegel Online 25. Oktober 2007). Dadurch wolle Microsoft die Anwendung ihrer Programm vereinfachen und sich individuell auf den Nutzer einstellen. Ein ausgereiftes Programm solle dann jeden Computernutzer mit Hilfe von bestimmten Gehirnstrommessgeräten, die er bei jeder Nutzung anschließen müsste, ausarbeiten, wann der Benutzer bei welchen Anwendungen Überraschung, Befriedigung und Frustration ausstrahlt und sich so nach dessen Wünschen selbst programmieren.<br />
Meiner Meinung nach sind solche Fortschritte nicht zu unterschätzen, genauso wie Metzinger denke ich, dass es nur noch eine Frage der Zeit ist, bis es dem Menschen möglich ist die Gedanken zu lesen. Dies wird allerdings den Menschen sehr stark einschränken, denn wie soll man fähig sein, sein normales Leben weiterzuführen, immer mit der Befürchtung im Hinterkopf, dass die Gedanken gelesen werden könnten? Wenn ich mir vorstelle, dass jede Person sofort wissen würde, was ich über sie denke oder jeder Lehrer wissen würde, dass die Schüler keine Ahnung von seiner Frage haben, dann würde ich nahezu Angst bekommen überhaupt zu denken. Denn Lügen gehören zum Leben einfach dazu. Ich spreche nicht von drastischen Lügen, die Leben kaputt machen, sondern von kleinen Notlügen, die uns oft gar nicht richtig bewusst sind. Schließlich lügt ein Mensch durchschnittlich bis zu 200 Mal am Tag. Außerdem stellt sich mir dann auch die Frage, wie Menschen in Zukunft Vertrauen und Sicherheit zueinander aufbauen wollen. Natürlich gibt es oft Zwickmühlen in denen man gezwungen ist anderen zu glauben was sie sagen, weil man es nicht nachprüfen kann, aber ist das nicht etwas sehr positives? Das man sozusagen zum Vertrauen gegenüber anderen gezwungen wird. Mit Lügendetektoren weiß man sofort, ob der Gegenüber die Wahrheit sagt oder nicht, ein wichtiger Grundstein, das Vertrauen ist allen Beziehungen dann also genommen. Auch wenn es manchmal sicher leichter wäre, z.B. bei Gerichtsverfahren eindeutig zu wissen, wer nun die Wahrheit sagt, wäre eine solcher Fortschritt sicher keine Erleichterung.<br />
Letztendlich kann ich sagen, dass ich lieber ein „Es war nett Sie kennen zulernen“ hören, als ein „Ich weiß Ihren Namen schon nicht mehr“ und es einfach manche Situationen gibt in denen ich die Wahrheit gar nicht wissen will.</p>

<p><em>Dancing in a Wheelchair</em> is the story of the Mutti family as they experience life as a family through the journey of two sons, Tim and Fred, with HIV / AIDS. The Muttis tell the story of the transition from life before diagnosis, through the first signs of the disease and finally to the loss of their two sons. Mixed throughout the book is factual information about the disease.</p>
<p><em>Dancing in a Wheelchair</em> is a heartfelt first person account of the events over several years. You can almost feel the passion of Fritz and Etta Mae as they experience grief, hope, sadness, anger and a wide range of emotions. I recommend this book to those who are experiencing HIV / AIDS as a part of their family as I believe that it would be a comfort and shine a light into the future.</p>

All that, was before I met Aicha. Aicha in Arabic means Living. Indeed, in her eyes we can see the flame of life that only true heroes have. In addition to her 8 children and husband, Aicha is living with an unpleasant guest everyday inside her weak body: AIDS.</p>
<p>With her pink traditional dress, which they call Tub in Sudan, Aicha was standing in front of 70 strangers to tell proudly her story with AIDS, during the UNDP HARPAS workshop for Independent Artists, Bloggers, and Journalists, which was held in Cairo from the 5th to the 8th May. “I didn’t commit any crime. I was operated for appendicitis, and they transferred to me blood infected with AIDS. I had to face my family, my children, and the whole society”, she said to her curious audience. In fact in many Arab countries a huge quantity of blood is still used without being well examined. Aicha was lucky enough to have an understanding husband, who supported and encouraged her to tell her story on television and in international meetings without any fear or shame, in such a conservative society full of taboos. Undeniably, “HIV/AIDS’ power is not in the Virus itself, but in the vicious circuit of fear and stigma linked to it”, as Doctor Ihaab Al Kharat from the HARPAS team has explained during the same workshop. In fact, AIDS is just like any other illness that we can live with without any risks if we take the right medicine at the right time.</p>
<p>Nowadays 39.5 million people worldwide are living with the Virus. In the Arab Region they are more than 460 000 people living with AIDS. Yet, I would like to question these figures given by the UNAIDS, because they are all based on government statistics. How can we imagine that a country like Syria only have 300 HIV/AIDS cases, without mentioning the whole Khalij region which doesn’t want to communicate any official figures on the issue? Another alarming figure is that only 5% of the declared AIDS cases in the region have access to treatment. Not because of luck in medicine, but because of the society taboos and of a coward suicidal discourse related to the Virus.</p>
<p>I was so impressed by Aicha that I decided to sit with her and have a long friendly talk. I was like a little child staring at this monument-like lady strong and confident in her 30s. She told me how her husband and she are living a normal sexual life by using condoms during their intercourses. Aicha also gave birth to a little girl, who doesn’t have the Virus, after following the right treatment that reduces the quantity of HIV in the blood during the pregnancy period. However, if science found a way to cope with the situation, the reaction of the doctors, who are supposed to be the most compassionate towards people living with AIDS, was very harsh on her. Once the medical staff learned about her case, they just put her in the quarantine and left her sinking in her blood and tears, shouting until the head of the baby came out.</p>
<p>When I’ve heard that story, I was so angry and disgusted at the same time. I cried fiercely and hugged Aicha. I could not describe that moment. I felt that she is a young woman just like me, and that all the stereotypes of the society disappeared. 48% of people living with AIDS in the Arab World are women, and Aicha is one of the few women who are coping with the Virus in a normal way. I feel I’ve found a hero made out of flesh and blood, who can inspire me in my daily life. For Aicha, and because I believe in life, I will go tomorrow morning to check my blood in one of the local centres, where I can get a free HIV/AIDS test. I hope you’ll do the same!</p>

Kiala at <a href="http://www.faceofthecookie.com">Face of the Cookie</a> wrote a post a while ago about how funny AIDS is and it reminded me of this story. Maybe I'll get a lot of shit for my post just like Kiala did. I hope so! But if you can't laugh at AIDS what CAN you laugh at? That statement alone should anger at least 20 people. AT LEAST. Annnnywaaay, about a year ago I used to live in Green Lake with my awesome roommate Jeni and we had quite the Blast. Our apartment was ok minus the old appliances and the fact that it was infested with ants. Lots and lots of ants. But it was big and it was in Green Lake and across the street from a cool bar so we put up with the damn ants and the inevitable ant traps that were everywhere. It was also pretty affordable...probably because of the ants. We moved into it almost two years ago to the day - right when the weather was getting sunny. The apartment building across from us had much nicer and Fancier apartments. We're talking new appliances, huge balconies, and it didn't look like it was about to fall apart. The apartment also had young girls who just sat on the balcony alternately sunbathing and having parties all while wearing cute clothes and drinking. I hated them. They were ALWAYS there. They didn't appear to have jobs and could afford an Awesome apartment and lots of booze and fashion-y dresses. So one night to make myself feel better my friend J and I decided they probably had STD's and that made everything alright. Sure I was overly jealous of their lives, but, hey, I don't have any STD's, so THERE! But we couldn't exactly go around and say things like, "Can you believe the dress that syphilis girl was wearing?!" or "I can't believe the girl with Crabs has yet another boy stalking her!" So we called them the High Five girls. Totally innocent to anybody who has no idea that in my mind High Five is slang for the AIDS. As in HIV...GET IT?! So now you all know what a horrible person I am.</p>

<title><![CDATA[Targeting cellular mechanisms for the treatment of HIV]]></title>

<content:encoded><![CDATA[<p>An article by Readinger <em>et al</em> entitled “<a href="http://www.pnas.org/cgi/content/abstract/0709659105v1">Selective targeting of ITK blocks multiple steps of HIV replication</a>” and published (free access) in the May 6th edition of the <a href="http://www.pnas.org/">Proceedings of the National Academy of Sciences</a> has been receiving a certain amount of press recently and for good reason.</p>
<p>Most therapeutic approaches to the treatment of HIV infection have been—and continue to be—built on the idea of attacking mechanisms of the virus itself. <a href="http://www.avert.org/introtrt.htm">Highly Active Antiretroviral Therapy</a> (HAART) is an example of this. However, antiretroviral resistant strains of HIV are appearing, thus reducing the efficacy of these treatments.</p>
<p>One potential path for increasing the arsenal of HIV treatments is to target cellular mechanisms that are essential to HIV infection. Indeed HIV, as is the case with all viruses, must hijack cellular mechanisms in order to reproduce itself. In their article, Readinger’s team reports on their research into a cellular protein called “Inducible T cell <a href="http://en.wikipedia.org/wiki/Kinase">kinase</a>”, or ITK for short. This cellular protein is essential, at least <em>in vitro</em>, for T-cell activation and appears to play a key role in several steps of HIV replication. Its inactivation, which they experimented using ITK-specific <a href="http://en.wikipedia.org/wiki/SiRNA">siRNA</a> or a chemical inhibitor with the charming name of BMS509744, may thus be of therapeutic importance.<br />

Their experiments indicate that ITK does indeed play an important role in HIV infection and replication and that its inactivation in CD4+ T-cells may hinder HIV infection.</p>
<p>They found that ITK was important at several steps in the HIV replication cycle:</p>
<ul>
<li><em>Viral entry</em></li>
</ul>
<p>ITK inactivation did not affect the production of <a href="http://en.wikipedia.org/wiki/CD4">CD4 receptors</a> or the <a href="http://en.wikipedia.org/wiki/CXCR4">CXCR4 (co)receptor</a>, and thus viral binding was unaffected. However, they found that viral entry was affected. This was because ITK is a necessary component of cytoskeleton rearrangement, something the HIV particle must induce via its <a href="http://en.wikipedia.org/wiki/Gp120">gp120 glycoprotein</a> to be able to enter the cell.</p>
<ul>
<li><em>Transcription from the HIV <a href="http://www.stanford.edu/group/nolan/tutorials/retcl_3_ltrs.html">long terminal repeats</a> (LTR)</em></li>
</ul>
<p><a href="http://en.wikipedia.org/wiki/Transcription_factor">Transcription factors</a>, such as NF-κB, need to bind to the LTRs of the viral genome to induce their transcription in activated T-cells. Using a HIV-specific luciferase reporter (a sort of biochemical signaler of transcription factor binding activity) the researchers found that ITK is an upregulator of HIV transcription and blocking it in activated T-cells reduced binding activity by 60-80% (as indicated by the luciferase).</p>

<li><em>Viral assembly and escape</em></li>
</ul>
<p>The team also looked at ITK’s role later in the viral cycle and found that in a state of overexpressed Gag proteins with active ITK, there was an increase in the creation of <a href="http://en.wikipedia.org/wiki/Virus-like_particle">virus-like particles</a>. They further explained that this potentiation did not seem to be dependant on kinase activity, but "in a manner dependent on protein interactions and membrane recruitment."</p>
<p>The also found that the use of ITK-specific siRNA after the establishment of infection reduced virion release by as much as 68% 2 days after treatment and that there were fewer infected cells (6 vs 18% (for controls) after 72 hours). However this effect was limited to 72 hours, which is a result of the transient nature of siRNA.</p>
<p>It is also worth noting that the researchers found that ITK did not seem to have a role in HIV reverse transcription or proviral integration.</p>
<p><strong>A few final points</strong></p>
<p>One of the questions that you would have the right to ask is, “hold on a minute, wouldn’t turning off a protein essential to T-cell activation be a bad idea?” Indeed, you generally want your T-cells to activate when stimulated by antigens. To respond to this question, the researchers underlined previous research, indicating that ITK deficient mice are still capable of mounting a defense against and clearing all viral challenges tested to date. So, although the T-cell response is partial, it is still sufficiently strong to fight off infections. It is this quality that makes ITK so interesting, in that its inactivation may hamper the progression of HIV, without having a major impact on the global T-cell mediated immune response.</p>
<p>Another question that you could ask is, “hold on another minute, although interfering with ITK function hampers HIV, it doesn’t stop it at 100%. Isn’t HIV going to simply evolve to get around the absence of active ITK? In the discussion the researchers are attentive to say that a cellular protein approach may be “less prone” to creating viral resistance. Indeed in the last sentence of their report they are careful to say that their results suggest, “that ITK inhibition provides a model for the study of cellular protein targets that affect HIV infection, which may be useful as part of a multidrug regimen directed against HIV.”</p>
<p>In other words, <em>prudence prudence prucence</em>. Indeed this is strictly an <em>in vitro</em> study in a relatively new field of research. The road ahead is still very long and HIV has proven itself as one real bastard of a virus, and I’m using a euphemism there. This isn’t the first “promising piece of research” and it certainly won't be the last. It is however an important step forward and a very interesting insight into HIV viral function and its dependence on this cellular ‘tool’ to assure its own reproduction.</p>

<content:encoded><![CDATA[<p>You know that television show by MTV called "<a href="http://www.mtv.com/ontv/dyn/next/series.jhtml"><span style="color:#999999;">Next</span></a>"? The show where you can date several people in one day and have the liberty to say "Next" when you want to see who's available in the chosen line up? It's a super cheesy show that, for some reason, makes you sit there and watch episode after episode on a Saturday when you really needed to do something else. Maybe I'm not doing a good job describing the show, but you get the idea.</p>
<p>Today, my office offered a chance to give blood. I, being a good citizen, sign up. I wait all day for my appointment, drink plenty of water and eat regularly so I would be ready. As I'm waiting to go through the screening process, I'm staring at all the awkward things in a blood-drive bus. Reclining chairs, weighers that look like something behind the butcher counter and plenty of needles, tubes, bags and bandages. As I'm zoning out watching the cool weigher rock back and forth to help fill the donor bag, I'm called to the back.</p>
<p>I get the finger pricked, I take the 20 something question test*, fill out all the right forms only to find out I'm 1.5 short of being able to give blood. What?! My iron was too low to allow me to give blood. As my nurse had me sign a form and what not, I was politely asked to leave. "You may take a cookie, juice and t-shirt if you want" was her last remarks to me.</p>
<p>As I left the bus slightly defeated, I walked the long walk of "your blood wasn't good enough" back to the building. I've always been able to give blood, but in that moment my blood was "Nexted" for some better, higher in iron blood. It was as if when I was walking into the building everyone was looking out their windows thinking, "she just got 'nexted'".</p>
<p>I feel your pain "Tom" from Rhode Island who's always in for a good time of basketball and fondue sampling. I feel you.</p>
<p>* I guess I'm more naive than I really thought, because as I was reading the rules I didn't realize gay men can NOT give blood. The rule specifically stated you can not give blood if you've had sex with a man, even once, since 1977. What? I know that was every one's initial fear and perhaps there's some scientific proof that it has been passed that way, but that's not the reason or the only way one can get HIV. Drives me crazy. For all they know I'm foot loose and fancy free with my body and could be carrying it. Luckily I'm not, but still way to discriminate.</p>

<p style="text-align:left;">Mr. McCain: “Well I think it’s a combination. The guy I really respect on this is Dr. Coburn. He believes – and I was just reading the thing he wrote– that you should do what you can to encourage abstinence where there is going to be sexual activity. Where that doesn’t succeed, than he thinks that we should employ contraceptives as well. But I agree with him that the first priority is on abstinence. I look to people like Dr. Coburn. I’m not very wise on it.”</p>
<p style="text-align:left;">(Mr. McCain turns to take a question on Iraq, but a moment later looks back to the reporter who asked him about AIDS.)</p>
<p style="text-align:left;">Mr. McCain: “I haven’t thought about it. Before I give you an answer, let me think about. Let me think about it a little bit because I never got a question about it before. I don’t know if I would use taxpayers’ money for it.”</p>
<p style="text-align:left;">Q: “What about grants for sex education in the United States? Should they include instructions about using contraceptives? Or should it be Bush’s policy, which is just abstinence?”</p>
<p style="text-align:left;">Mr. McCain: (Long pause)  “Ahhh. I think I support the president’s policy.”</p>
<p style="text-align:left;">Q: “So no contraception, no counseling on contraception. Just abstinence. Do you think contraceptives help stop the spread of HIV?”</p>
<p style="text-align:left;">Mr. McCain: (Long pause) “You’ve stumped me.”</p>
<p style="text-align:left;">Q: “I mean, I think you’d probably agree it probably does help stop it?”</p>
<p style="text-align:left;">Mr. McCain: (Laughs) “Are we on the Straight Talk express? I’m not informed enough on it. Let me find out. You know, I’m sure I’ve taken a position on it on the past. I have to find out what my position was. Brian, would you find out what my position is on contraception – I’m sure I’m opposed to government spending on it, I’m sure I support the president’s policies on it.”</p>

<p>Wednesday is hump day, the day that separates where your work week starts from when you work week ends. I thought what a perfect day to take a break and do something quick and easy that will make a positive difference. This day will be reserved for petitions and causes.</p>
<p>Our first lucky "Hump Day" winner is <a title="ONE" href="http://www.one.org/" target="_blank">ONE</a>. I first heard of these guys reading <a title="Ben and Jerry's" href="http://www.one.org/benjerry/" target="_blank">Ben and Jerry's</a> email newsletter. They have partnered with the organization to gain more exposer for their cause. What ONE does is raise public awareness about the issues of global poverty, hunger, disease and efforts to fight such problems in the world's poorest countries.</p>
<p>In their own words, "<em>ONE believes that allocating more of the U.S. budget toward providing basic needs like health, education, clean water and food would transform the futures and hopes of an entire generation in the world's poorest countries.</em></p>
<p><em> ONE is nonpartisan; there's only one side in the fight against global AIDS and extreme poverty. Working on the ground in communities, colleges and churches across the United States, ONE members both educate and ask America's leaders to increase efforts to fight global AIDS and extreme poverty, from the U.S. budget and presidential elections to specific legislation on debt cancellation, increasing effective international assistance, making trade fair, and fighting corruption.</em>"</p>

<p>It was a good morning. I got up on time and headed to the clinic for my protocol appointment. It went very quietly and without any more drama, thank God...</p>
<p>So here are the numbers:</p>
<p>Viral Load: Undetectable<br />
CD4%        37%<br />
CD4's        1073 ... A new highest number<br />
CD8%         43%</p>
<p>My financial Aide came through this morning and they approved my file. YAY!!!</p>
